3. Consultation Appointment Bookings

3.1 Payment of the consultation fee reserves your requested appointment time subject to doctor availability. Confirmation will be sent via SMS or email within one business day.

3.2 If your requested time slot is unavailable, you will be offered alternative appointment times or a full refund.

3.3 Cancellations made at least 48 hours before the scheduled appointment are eligible for refund minus applicable processing fees. Cancellations within 48 hours and no-shows are not refundable.

3.4 UHSL may cancel appointments due to doctor unavailability or operational circumstances. In such cases, you will be offered rescheduling or full refund.

3.5 The consultation fee covers the appointment only. Diagnostic tests, procedures, and medications are billed separately.

3.6 Patients arriving more than 15 minutes late may forfeit their appointment without refund, subject to doctor availability.

4. Diagnostic Packages and Health Checkups

4.1 Payment confirms your purchase of the selected diagnostic package. Packages must be utilized within the validity period specified at time of purchase.

4.2 Appointment scheduling for diagnostic services is subject to availability. You will be contacted to schedule your tests after purchase.

4.3 Certain tests require preparation such as fasting or specific timing. Failure to follow preparation instructions may require rescheduling without refund of the package cost.

4.4 Test reports will be delivered within the timeframe communicated at time of booking. Delivery methods may include physical collection, courier, email, or patient portal access.

4.5 If a specific test within a purchased package becomes unavailable, you will be offered a comparable alternative test or partial refund for that component.

4.6 Diagnostic packages are non-refundable once any test within the package has been conducted.

5. Medical Records and Reports

5.1 Requests for medical records, diagnostic reports, or document copies require proper patient identification and authorization.

5.2 Records will be delivered within the specified timeframe via the selected delivery method (physical collection, courier, digital copy).

5.3 Fees for record requests are non-refundable once processing has begun.

5.4 UHSL provides records in available formats. Format conversion requests may incur additional charges.

6. Pharmacy Products

6.1 Prescription medications require valid prescription submission. UHSL reserves the right to verify prescriptions with the prescribing physician.

6.2 Orders are subject to product availability. If items are unavailable, you will be contacted for substitution approval or partial refund.

6.3 Delivery timelines and geographic coverage vary by location. Delivery fees, if applicable, will be displayed at checkout.

6.4 Pharmaceutical products are non-returnable once delivered except in cases of product defect, damage during transit, or dispensing error by UHSL.

6.5 Controlled substances and certain medication categories may have additional verification or delivery restrictions.

6.6 Storage, handling, and usage of medications are the responsibility of the purchaser. Follow all product labeling and physician instructions.

7. Health Packages and Memberships

7.1 Health packages include specified services valid for the stated duration from date of purchase.

7.2 Services included in packages are subject to prior appointment scheduling and availability.

7.3 Packages are non-transferable between individuals unless explicitly stated as family or transferable packages.

7.4 Unused services within a package expire at the end of the validity period without refund or credit.

7.5 Package terms, included services, and validity periods are specified at time of purchase and cannot be modified retroactively.

8. Payment Processing

8.1 All payments are processed through secure third-party payment gateways. UHSL does not store complete credit card numbers, banking credentials, or mobile financial service PINs.

8.2 Payment confirmation constitutes acceptance of the relevant service terms.

8.3 Payment gateway processing fees, if applicable, are non-refundable.

8.4 For payment disputes, chargebacks, or transaction failures, contact your payment provider directly.

9. Refund Processing

9.1 Approved refunds are processed within 7-10 business days from refund approval date.

9.2 Refund timelines depend on the original payment method. Card refunds may take longer than mobile wallet refunds.

9.3 Processing fees, if deducted at time of purchase, are non-refundable.

10. Service Modifications and Cancellations by UHSL

10.2 If UHSL cancels a booked service due to operational circumstances, you will be offered rescheduling or full refund of amounts paid for that specific service.

10.3 Pricing displayed at time of purchase is honored for that transaction. Subsequent price changes do not affect completed purchases.

11. User Responsibilities

11.1 Provide accurate information when booking or purchasing services. UHSL is not responsible for errors resulting from incorrect user-provided information.

11.2 Maintain confidentiality of patient portal login credentials.

11.3 Arrive on time for scheduled appointments and follow preparation instructions for diagnostic services.

11.4 Use pharmacy products according to prescriber instructions and product labeling.
